Electric Efficiency
Electric Efficiency Program at a glance: • Provides funding for energy-efficiency projects at commercial and industrial facilities.
• Projects must provide reliable, measurable, and verifiable savings.
• An application must be submitted and approved and a Project Implementation Agreement signed before materials are ordered and before demolition or installation has begun.

Application and Reporting Forms
1. Project Application (PA) (Excel format)
Application may be made either by entering project information directly into the Excel version of this form on your computer and submitting the file as an email attachment or by requesting a hard copy from Platte River, completing it by hand, and sending it in by mail or fax.2. Customer Agreement (Acrobat format)
This form is required only if a third party (e.g., an energy services company or contractor) is acting as the program participant and signing the Project Implementation Agreement. In this case, the Customer Agreement is signed by a representative of the utility customer that is hosting the project. Note that this page is also contained as a page in the Project Application spreadsheet.3. Project Implementation Agreement (Acrobat format)
This is the agreement that details the mutual understanding regarding funding to be committed and the specifics of the work for which funding is to be paid. To be completed and signed by both you and Platte River after the Project Application has been approved. (4 pages)4. Request for Payment (Acrobat format)
Submit this form as an application for payment upon completion of the project installation.a. Form W-9 – Request for Taxpayer Identification Number and Certification (Acrobat format): This form must be completed and returned with your Request for Payment.
b. Affidavit of Lawful Presence in the United States (Acrobat format): This form is required by state law if you are an individual or sole proprietor (i.e., not a corporation or partnership) applying for utility incentives. This must be submitted along with your Request for Payment.
4. Customer Agreement (Acrobat format)
Include this form with the Project Application only if a third-party service provider (the “Participant” in the Electric Efficiency Program) will be signing the Project Implementation Agreement with Platte River. That is, the company applying for funding is not a customer of Platte River. The Customer Agreement assures Platte River that the customer understands the funding arrangement between the Participant and Platte River, and that access to customer facilities will be granted for measurement and verification activities if necessary.
