Cooling Rebate Application Form (Adobe Acrobat format 4.0 and above)

• Form W-9 – Request for Taxpayer Identification Number and Certification: This form must be completed and returned with your application.
• Affidavit of Lawful Presence in the United States: This form is required by state law if you are an individual or sole proprietor (i.e., not a corporation or partnership) applying for utility incentives. This must be completed and returned with your application.
